New patient appointments are currently scheduling 2-3 weeks out from date of enrollment. Availability will be assessed on a rolling basis. Potential patients who have completed the enrollment form will be contacted for a Meet & Greet call with Dr. Querns. Completing the enrollment form does not guarantee a spot in the practice, but it does place you in line for the next available membership opportunity. 

No membership billing will occur unless you are seen for an establish visit. All memberships are initiated with a non-refundable enrollment fee of $150 and sustained by a monthly age-based membership ($85-145/month).
