As of May 2026 the practice is now on a wait-list. Availability will be assessed on a rolling basis. Potential patients who have completed the enrollment form and joined the waitlist will be contacted if a spot is available. Completing the enrollment form does not guarantee a spot in the practice, but it does place you in line for the next available membership opportunity. 

No billing will occur unless you are contacted to schedule an establish visit. All memberships are initiated with an enrollment fee of $150 and sustained by a monthly age-based membership ($70-130/month).
