Pediatrics: 0 - 17 years

$70/month

From well visits and sick care to developmental support and vaccine guidance, your child receives comprehensive care in a comforting, judgment-free environment.

Includes:

Unlimited office visits

Text/call access to your doctor

School, sports, and camp physicals

Preventive care and chronic condition management

Newborn through teen transitions

🍼 Perfectly pairs with our SOAR Nestling Care for newborns and toddlers!

Adult: 18 - 60 years

$100/month

Whether you're navigating early parenthood, career stress, or hormone changes, this membership gives you quick access, compassionate care, and personalized wellness planning—all in one place.

Includes:

Unlimited visits with your physician

Annual physicals + lab coordination

Chronic disease care (e.g., diabetes, thyroid, PCOS, anxiety, hypertension)

Same-day or next-day appointments

Nutrition, lifestyle, and preventive medicine support

Osteopathic manual therapy (if applicable)

Seniors: 60 - 110 years

$130/month

We take time to truly listen. Our senior plan supports you through aging with strength and dignity—focusing on prevention, mobility, independence, and peace of mind.

Extended, unrushed visits

Cognitive and functional health check-ins

Coordination with specialists and family caregivers

The Fine Print, Upfront.

All memberships are initiated by a one time enrollment fee of $150.00 per person.

Families sharing one household will have their enrollment fees capped at $300.00. 

In addition, families sharing one household will have their membership fees capped at $300/month. In order to be eligible for this, at least one parent and one child must be enrolled at all times. 

Individuals who elect to leave the practice will only have coverage for 30 days after discontinuation of membership. Non-payment of membership fees constitutes discontinuation of membership. 

If an individual leaves the practice, and elects to return, the enrollment fee will be required prior to re-starting membership.
